The way we introduce people to Network Care and our office is through a comprehensive initial visit. Your visit will start with a consultation with Dr. Matthew. He will take time to find out what's happening in your body and in your life and discuss with you what your goals are. This gives him the opportunity to give you information about Network Care that really makes sense to you. You will then receive a full spine and nerve system evaluation. Your visit continues with a Network session, giving you the opportunity to experience the work for yourself. If you wish to continue on, Dr. Matthew will give you his recommendations for care. You will be at the office about an hour and a half for your initial visit.

Before you come in for your initial visit, please download, print, and complete the forms below. Or if you prefer, you can request that the paperwork be mailed to you. Some of our questions may be different than those you have encountered before, so we really appreciate your taking the time to fill out the paperwork completely. This will assure that you receive the most from your initial visit.
